Patient-Centered Medical Home
Great news, Bayside Pediatrics will be implementing a Patient-Centered Medical Home (PCMH)!
This approach involves making our office the point person around which all care revolves. Developed by the help of the American Academy of Pediatrics, the medical home model involves making our office accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective to all our patients.
We believe PCM is the best way to organize and deliver primary healthcare, because it provides both a framework and an opportunity for you to develop trusted, long-term relationships with your child's doctor and team. Your team coordinates services at every point of the healthcare spectrum— prevention, hospitalization, specialty care, community services and supports. The model also focuses on not only the physical, but also the mental/ behavioral health of our patients.
With a patient-centered medical home approach and care, both routine and urgent, is highly acessible to you, your child and your entire family. Good communication is emphasized. PCM also takes full advantage of the latest information technology to prescribe, commnicate, track test results, obtain clinical support information monitor performance and obtain visit summaries about care obtained outside the practice. PCMH model will also focus on providing evidence based care and support patient and patient's family self-management of their health.
